Use the punch included to make holes on the strip of paper in the right place on the staff. Then simply insert through the slot and crank. Please note that it is a hand cranked mechanism; you have to keep winding it in order to hear the music. You need to have musical writing knowledge to be able to make your own melodies, or at least enough competency to correctly copy sheet music of the tune you want.
